Description

As a Care Coordinator, you will be managing an innovative Chronic Care Management program (ON SITE IN LEWISVILLE, TX) leveraging the best-in-class virtual care and Remote Patient Monitoring (RPM) technology/devices to improve health outcomes for patients enrolled.

Key Responsibilities:

  1. Remotely providing basic patient coaching and care to improve patient outcomes 

  2. Develop a care plan with the patient, family/caregiver(s), and provider 

  3. Monitor patient outcomes, adherence to care plans, evaluate effectiveness, monitor patient progress in a timely manner, and facilitate changes as needed 

  4. Coaching and educating patients on improving their Chronic Conditions, preventive care, and physician directives 

  5. Telephone outreach to English and Spanish-speaking patients 

  6. Facilitate CCM and RPM program enrollment 

  7. Concise and accurate documentation 

  8. Cultivate and support the primary care providers with timely communication, inquiry follow-up, and integration of information into the care plan regarding transitions-in-care and referral

  9. Work with the clinical team in the practice partner to launch and scale a successful Virtual RPM / Chronic Care Program

  10. Patient intake into the program – Reviewing patients and identifying patients that fit the inclusion criterion for Remote Monitoring

  11. Answering questions about our Chronic Disease Practice from prospective patients

  12. Actively enrolling patients into the program at the clinic location

  13. Discussing the RPM program with patients and onboarding them into the program

  14. Training patients on the use of at-home vital sign monitoring equipment and mobile application

  15. Keeps supplies on hand by inventorying stock and liaising with RPM partners to ensure adequate supply

  16. Assigning RPM kits to patients onboarding into the program

  17. Off-boarding patients when necessary, from the program and preparing their devices for the next patient

  18. Connecting with patients in the Chronic Care Management (CCM) cohort via phone and/or video telemedicine once a month or when they have questions

  19. Triage of patient problems and notifications identified by the RPM technology

  20. Generate reports and notes about the patient's condition and escalate to the appropriate clinical resource within the clinic

  21. Outstanding interpersonal skills especially with the elderly and a customer service attitude is an absolute requirement for this role

  22. Constantly explaining and selling the benefits of the program to internal and external stakeholders

Qualifications

  1. Foreign Medical Graduate Certification (FMG) or Medical Assistant (MA) or Certified Nurse Assistant (CNA) or pharmacy intern - preferred

  2. Bilingual (Spanish) preferred

  3. Excellent Communication and customer service skills

  4. Knowledgeable of using computer applications

  5. Understanding of US healthcare system

  6. Criminal background checks will be conducted for successful applicants
